The State of Connecticut, Department of Developmental Services (DDS) -North Region is recruiting for a full-time, Clinical Nurse Coordinator assigned to the Individual and Family Support Division based out of the Willimantic office.

This is a full-time (35 hours per week) first shift position. The work schedule is Monday through Wednesday: 9:00 AM to 4:30 PM, Thursday and Friday: 10:30 AM to 6:00 PM. Regular days off are Saturday and Sunday.

Requirements for this role:
  • Must possess and retain a valid Motor Vehicle Operator's License
  • Will be required to travel
  • Must possess a current occupational license as a Registered Nurse in CT

PURPOSE OF JOB CLASS (NATURE OF WORK)

In a state facility this class is accountable for coordinating the various facets of clinical programs involving multiple units OR for serving as liaison between the facility and community programs and placement settings.

EXAMPLES OF DUTIES

Serves as a resource/consultant to individuals living independently or with their families in the community in the Northeastern corner of the Region, as well as to DDS staff and community agencies and practitioners. May accompany individuals on appointments and ensures they are in the least restrictive environment; assures continuity of care to prevent recidivism; Makes home or other site visits; conducts nursing plans and assessments and participates in Abuse Investigation Division (AID) investigations, including assisting with Immediate Protective Service Plan and Protective Service Plan (IPSP, PSP) responses; assesses individuals in temporary nursing home/rehab placements; maintains accurate written and electronic data and reports. Provides direct and indirect nursing oversight at DDS Respite Centers in Putnam and Storrs; assures staff in Respite and Individual and Family Support (IFS) are maintaining medication administration certification and oversees the overall nursing component of the youth licensed summer camp at Camp Quinebaug in Killingly. Respite duties may include conducting family pre-visits; attending team meetings; assisting in formulating guest groupings; collecting guest’s health and medical information prior to admissions; obtaining, maintaining accurate Kardex’s from licensed practitioner’s orders; reviews diet and nutritional needs of individuals; nursing delegation of duties to med certified staff; reviewing guest’s health plans; participating in guest admissions/discharges as needed; staff training and in-servicing. Works closely with Respite Center Staff and the IFS Resource Teams; may clinically supervise direct care staff in execution of clinical programs; provides admission and discharge nursing assessments including plans for continuing care; assures nursing care programs are carried out thoroughly and on schedule; investigates medically related problems which have been referred by staff, other team members or the Department; maintains records of drugs used, stored and secured; provides for nursing services to comply with Intermediate Care Facilities and Community Living Arrangements (ICF, CLA) regulations; provides nursing input to interdisciplinary health care plans; provides on-going training to licensed and non-licensed staff to ensure their competence in providing safe care and the ability to protect the health care of individuals ; completes documentation, including electronic case notes, data reports, records and other information concerning services and care; ensures that resources are available to provide quality services; may develop and conduct in person or social media based in-service training for individuals, staff or families; will act as an important member of the Team to provide medical information and attend meetings and make reports on health status. Participates in Regional On-Call Nursing system. May cover temporary assignment in Public CLA’s. Must be flexible to meet the needs of individuals and families in providing services and to provide seasonal summer coverage at Camp Quinebaug; performs related duties as required. 

KNOWLEDGE, SKILL AND ABILITY

Knowledge of nursing principles and clinical therapeutic models of individual care with emphasis on interdisciplinary team approaches; knowledge of individual treatment planning including impact of medical insurance program requirements for reimbursement; knowledge of structure of services for individuals and their families in institutional and community oriented settings; knowledge of aftercare facilities; considerable interpersonal skills; oral and written communication skills.

MINIMUM QUALIFICATIONS - GENERAL EXPERIENCE

Three (3) years of professional nursing experience.

MINIMUM QUALIFICATIONS - SPECIAL EXPERIENCE

One (1) year of the General Experience must have been at the level of Head Nurse working with the type of individuals serviced by the facility.

DEPARTMENT OF DEVELOPMENTAL SERVICES: One (1) year of the General Experience must have been providing consultation and evaluating the assessment of nursing services provided to individuals with intellectual disabilities.

MINIMUM QUALIFICATIONS - SUBSTITUTIONS ALLOWED

  1. A Bachelor's degree in nursing may be substituted for one (1) year of the General Experience.
  2. A Master's degree in nursing may be substituted for one (1) additional year of the General Experience.

PREFERRED QUALIFICATIONS

Preference will be given to candidates with the following experience:
  • Working with both children and adults with Intellectual and/or Developmental Disabilities
  • Interacting with and explaining information to individuals in a fast-paced environment that involves considerable phone and email contact
  • Using Microsoft Office Suite including Outlook, Word, and Excel
  • Coordinating supports for discharge planning of individuals from hospitals or rehab facilities to own or family homes
  • Training and overseeing medication administration certification for non-licensed staff
  • Working in licensed youth camp settings

SPECIAL REQUIREMENTS

  1. Incumbents in this class must possess and retain a license as a registered nurse in Connecticut.
  2. Incumbents in this class may be required to travel.
