Introduction

"he Department of Developmental Services - South Region - Self Determination Division has a job opportunity for a DS Case Manager Trainee position based out of the Wallingford Office. This is full-time thirty-five (35) hour per week position. The work schedule is Monday-Friday on first shift; 8:30a.m. to 4:00 p.m.; however, must be flexible in order to meet the needs of the agency.

PURPOSE OF JOB CLASS (NATURE OF WORK)

In the Departments of Developmental Services and Social Services, this class serves as a means to participate in a formal training program to acquire professional experience, knowledge and skills for appointment to a designated Developmental Services Case Manager classification and to become eligible for a Qualified Intellectual Disability Professional certification/designation.

EXAMPLES OF DUTIES

Receives training and performs activities related to individual services plans for conformity with federal and/or agency standards; assists, coordinates and monitors the delivery of waivered services to ensure the federal requirements for reimbursement are met and maintained; assists in the preparation and monitors completion of appropriate reimbursement documentation; assists in the development of social service evaluations and service recommendations; oversees and supports continuity of care and waiver benefits; monitors development and ensures maintenance of individual files including required documentation; provides information and support to individuals and families in obtaining and maintains social services benefits to ensure continued waiver eligibility; assists in providing guidance and support to individual and/or family members in locating appropriate providers in the community; participates in interdisciplinary/planning support team meetings to develop, review and/or modify individual service plans; assists and coordinates a variety of individual services and ensures service delivery; assists with identifying the need for evaluations, scheduling of evaluations and monitors completion; monitors implementation of specific program recommendations; ensure legal and financial documents are completed in a timely manner; assists and participates in other individual related activities; performs related duties as required.

KNOWLEDGE, SKILL AND ABILITY

Considerable knowledge of services available to persons with intellectual disability; knowledge of residential programs for persons with intellectual disability, knowledge of interdisciplinary approach to program planning; knowledge of intellectual disability, causes and treatment; considerable ability to understand and translate clinical findings and recommendation into program activities and develop realistic program objectives; ability to collect and analyse large amounts of information; ability to utilize computer software; interpersonal skills; oral and written communication skills.

MINIMUM QUALIFICATIONS - GENERAL EXPERIENCE

The General Experience is defined as one of the following:

  1. A Bachelor's degree that meets the eligibility criteria for certification/designation as a Qualified Intellectual Disabilities Professional (QIDP) as set forth in federal regulations and interpretive guidelines.
  2. A Master's degree that meets the eligibility criteria for certification/designation as a Qualified Intellectual Disabilities Professional (QIDP) as set forth in federal regulations and interpretive guidelines.

NOTE: A degree that meets the eligibility criteria for certification/designation as a Qualified Intellectual Disabilities Professional (QIDP) is a degree in the field of human services, healthcare or education including but not limited to nursing, psychology rehabilitation counseling, special education or sociology.

PREFERRED QUALIFICATIONS

• Experience in implementing the DDS Mission

• One (1) year of experience in participating in Person Center Planning 

• One (1) year of experience with community resources and assisting individuals to live and work more independently 

• Experience in empowering individuals to advocate for themselves 

• Experience with self-directed individuals managing their own staff  

• Proficiency in MS Office, specifically Excel, Word, and Access
