Reinstatement:
Pursuant to Civil Service Rule 1744: On the request of a department head and approval of the Commission, a regularly appointed employee who resigned in good standing may, within three years after the effective date of his/her resignation, be reinstated in a position in the class from which he/she resigned or in a position in another class for which he/she may be eligible as determined by the Commission. Appointments by reinstatement following resignation are subject to the probationary period established for the class. You must have been a classified County employee in the same job code or in a job code at the same salary maximum and left the County in good standing. In addition, you must meet the minimum and special qualifications of the specific position.

HEALTHY HOMES DEPARTMENT
Established in 1991, the Alameda County Healthy Homes Department has become a national leader in childhood lead poisoning prevention and healthy homes programs, combining health, environmental and residential hazard reduction services under one umbrella.
THE VACANCY The Community Development Agency Healthy Homes Department currently has an opening for Registered Nurse II-PHN option. This position provides Nursing Case Management to families of children with elevated blood lead levels and requires a Public Health Nurse Certificate to conduct home visits (in-person & remote) with families. The nursing process is utilized to provide health education on lead poisoning prevention, lead source identification, monitoring of blood lead levels, developmental evaluation and monitoring, coordination with medical providers, linkages to ancillary services and resources, advocacy, and evaluation of health outcomes. This position works with a multidisciplinary team of paraprofessionals in the environmental and housing sectors within the Department to address the housing-based aspects of the child’s lead poisoning for best health outcomes.
SPECIAL REQUIREMENT: In addition to meeting the minimum qualifications below, these positions require possession of a current and valid Public Health Nursing (PHN) Certificate issued by the California Board of Registered Nursing.
The Alameda County Healthy Homes Department's unique multidisciplinary approach serves the community in order to eliminate environmental lead contamination, prevent childhood lead poisoning and improve health outcomes by addressing housing problems. The Program provides case management of lead poisoned children, community outreach and education, training, lead hazard reduction services, healthy homes interventions, and consultations. THE POSITION
Registered Nurse II positions are located in the Health Care Services Agency, Social Services Agency and Community Development Agency. Registered Nurse IIs provide a variety of nursing care services, case management, treatment plan, teaching, counseling and referral to clients and families in a home and/or community setting, provide nursing assessment, planning, intervention and evaluation; coordinate care with other health care providers, professionals and other agencies and community representatives; assist in the prevention of communicable diseases; and perform related duties as required.
