Under general supervision to implement and supervise the day-to-day post discharge outreach program, an activity of the Every Child Counts program, in designated County hospitals and to perform other related duties as required.


This is a first level supervisory class found in the Every Child Counts program of the Alameda County Children and Families Commission. The incumbent of this classification may be stationed at one of several County hospitals. This position reports to the Family Services Director. It is distinguished from the Hospital Outreach Coordinator in that the latter does not have supervisory responsibility.

NOTE: The following are the duties performed by employees in this classification. However, employees may perform other related duties at an equivalent level. Each individual in the classification does not necessarily perform all duties listed.

1. Supervises the abstracting of prenatal, birth, and postnatal data from hospital records as well as the collection of required information and enrolls families in program; transmits information to public health nurses conducting home visits.

2. Supervises the coordination of home visit program activities with other hospitals, public health nurses, health plans, social services, health providers and other related programs.

3. Meets with pregnant mothers and their families touring the hospital and orients them to the Every Child Counts program.

4. Acts as a liaison between program participants and service providers.

5. Develops program plans for optimal health and social outcomes.


The equivalent of four years of full time hospital, case management or medical program experience performing duties such as: intake, peer counseling, obtaining personal history data, needs assessments, program screening, placement and referral.

(An Associate of Arts Degree from an accredited college in health services, social science, or related field may be substituted for two years of experience.)
