DEFINITION

Under general direction, directs operations of the San Joaquin General Hospital Medical Social Service Division; and does related or other work as required in accordance with Rule 3, Section 3 of the Civil Service Rules.

CLASS CHARACTERISTICS

This is a single position class in the Department of Health Care Services. The incumbent manages staff who provides medical social services, including discharge planning, to Health Care Services patient and families and participates in the provision of services. Direction is received from a Senior Deputy Director.

TYPICAL DUTIES

  • Plans, organizes and directs staff assigned to provide medical social services to patients and families; coordinates activities of the Medical Social Services Division.
  • Supervises and participates in the provision of medical social services which include:

a. Compiles extensive social case histories for use by medical staff in diagnosis and determination of treatment.

b. Participates in diagnostic and technical staff conferences with other hospital disciplines for treatment planning and post discharge continuing care; interprets recommendations to patients and families.

c. Evaluates and assesses the environment a patient will be released to prior to release to determine the suitability of the environment; recommends modifications, supplemental assistance and follow up plans.

  • Selects, assigns, trains and evaluates subordinate staff.
  • Maintains liaison between the SJGH Medical Social Services division and representatives of other social service agencies and other County departments.
  • Conducts research for special projects; prepares correspondence and reports; makes oral presentations; prepares and monitors division budget.
  • Develops policies and procedures; interprets and explains policies, procedures, rules and regulations to patients and families.
  • Coordinates division activities with the Utilization Review process to ensure appropriate discharge planning and continuity of care.
  • Provides staff education and training; disseminates information on programs; addresses public and private groups regarding SJGH social services.

MINIMUM QUALIFICATIONS

Education:  Master’s Degree in Social Work.

 

License:  Possession of a valid California license as a Clinical Social Worker (LCSW).  Possession of a valid California driver’s license.

 

Experience: Three years of progressively responsible post MSW social casework experience in a clinical, treatment or social treatment setting.

KNOWLEDGE

Principles and practices of social work including casework methods and techniques; principles and practices of discharge planning, medical social services provided by other public and private agencies in the area; federal, state and local public assistance programs; principles of personnel management and effective supervision; principles of sociology psychology, public health and social casework; principles of human development and behavior.

ABILITY

Plan, organize, coordinate and direct the work of others; prepare budgets; communicate effectively, orally and in writing; develop skill in utilizing community resources within the justice, social service, health and education systems and the community at large; establish and maintain effective working relationships with others.
